What would cause a swollen and obstructed kidney (no stones).?

I have a swollen and obstructed right kidney but after a CT and IVP they can not find any stones. Tomorrow morning I am scheduled for a cystoscopy with stent placement, but I still haven't been told what the obstruction could be. Any ideas?
The urologist said my ureters are normal size, but my kidney is severely swollen and obstructed. He is out of town, so his nurse called to tell me I needed this procedure done asap, and another doctor in the practice would be performing it.. I'm just confused as to what the obstruction is..

You have a procedure tomorrow and they haven't told you what the etiologies of your dilatation might be? Or your narrowing/obstruction? Which is it?
Some stones do not show up on CT.
If they are placing a stent, you might have a narrowing within your ureter, not an obstruction/object at all.
